Quickly create a form to use as lead generation or just to capture information from a user. The instructions below will help you get started as quickly as possible.
Note: Questions are divided up by sections. So if you add your questions across three sections then the user will have 3 pages of questions to fill out.
1. Login to Member Pages and Click on Courses & Engagement --> Forms and then Add External Form.
2. Enter a Name and Description for the page (These are for internal use only / Your users won't see them).
3. Enter a Title and Sub Title(optional) for the section. This section will be immediatly visible when the page loads.
4. Click on "Add Section".
5. Click on "Add/Edit Questions" which appears below.
6. Add Your First Question inside the Question box and the Note (optional).
The note field will be a line between the question and the answers. It is good to say something like: "Please select all that apply."
7. Select the type
Single means the user can select only one answer
Multiple means the user can select many answers
Text means the user can enter in one line of text (short answers, names, etc...)
Textarea means the user can enter into a larger area (for paragraphs etc...)
8. Check the "Required" Box if it is required for the user to add an answer.
Note for type Multiple it will be required for them to select at least one box and for text fields it will validate if the value is not blank.
9. Click on "Add Question"
10. Repeat Steps 6 - 10 to add as many questions as you need to the section.
11. Click on "Add/Edit Answers" for a specific Question.
12. Add an "Answer", "Value", and "Points to your question.
Answer is what the user will see
Value is any value you would like to see on the report (For example if the question is "Yes, I finished my homework today" then you could simply have the value be "yes"). By default it is copied from the answer.
Points are how many points the answer is worth if the select it (primarily for quizes)
13. Click on Select / Add Image to your question if you would like to add an image to it (Note this only applies to single and multiple types).
14. Click Add Answer
15. Repeat Steps 12-14 until you've added all the answers to that question.
16. Click Save Question
17. Click Save Section
18. Repeat Steps 3 - 17 for any additional sections you would like to add.
19. Enter a Pass Score. This applies to quizes. If you are simply creating a form you can enter 0 or leave it blank.
20. Enter a Pass URL. This is where the user will go after they finish filling out the quiz if they reach or score more than the pass score (or it is empty).
21 Enter a Fail URL. This is where the user will go if they score less than the pass score.
22. Click Save Form. You can then click on "Preview" to get the page url and preview the page.
